To create an application for your organisation, please complete below form. The agency head and the proposed team members will receive an email for the next step.
Please note that all information input must be identical to the official documents, such as Hong Kong Identification Card or Certification Document for a charitable organisation which is exempted from tax under section 88 of the Inland Revenue Ordinance (Cap. 112).
* For organisations running multi-disciplinary services (i.e., social services, medical services etc.), please only include staff of the Social Services Division.
